There is free parking in the rear of the building. While, there are several ways to enter the lot, the easiest is to enter from Lincoln coming from the north, just after Arby's. Do I need to bring anything?
  • Your prescription: Please have it in hand or, if you asked your MD to fax it, call and confirm that we have received it. By law, we can not treat you without a diagnosis, and MD's typically take several days to get around to faxing prescriptions.
  • Your insurance card.
  • Your completed paperwork: Please download, print and fill out all four Patient Forms from the website (the password is PTW in all caps).

How long will my appointment be?
For your first visit, expect to be here for up to 90 minutes. Each subsequent visit can last anywhere between 45 and 75 minutes.   back to top
What if I need to cancel or change my appointment?
Call the office: (310) 260-9039. If we are not given at least 24-hours notice, we charge a $50 cancellation fee. Why? After three cancellations, the fee increases to $75. No-show fees are not covered by your insurance. We require this much time so that we may attempt to fill your scheduled appointment time. If we are able to fill your vacancy, even if you gave less than 24-hour notice, we will waive the fee.   back to top
Are you a preferred provider for _________?
We are preferred providers with nearly every insurance company. Some of these companies include Aetna, Blue Cross, Blue Shield, CIGNA, First Health/CCN, Health Net, Medicare, Pacificare, United Health Care and many others. We are also preferred providers with several administrative groups (administrative groups administer benefits for a multitude of insurance companies). The administrative groups we belong to include Beech Street, Capp Care, Choice Care and PHCS. Look on your insurance card: if it has one of these logos on it, then we are 'networked' with your plan. If you're not sure if we are preferred providers for your insurance company or administrative group call us and we will find out for you.  back to top
